Genuine WWII German medical tag. The form was to be filled with the sick or wounded soldier's basic information (identification, diagnosis, treatment) and then tied to his clothes thanks to the string intended for this purpose. A tag booklet was to be carried by every medic. This is the specific version intended for gas casualties.
